And he ignored a huge negative trait that Orochimaru had which developed into a huge problem for the village Hiruzen claimed to love. On the flip side, Hashirama dealt with Madara the moment Madara made his move against the village.

And before that Hashirama never claimed to have noticed any malice etc. The nail in Hiruzen's coffin came out his own mouth - he noticed Oro had problems and ignored said problems in favour of turning the genius kid into a weapon. Hashirama was a bit off beat and naive but he would know if something is off. Him acting on it is a different story because they formed a friendship. And yet he didn't know that something was off, suggesting that you are incorrect. Hashirama saw the absolute best in people - he honestly was going to give the other Kage weapons of mass destruction for free until Tobirama stepped in.

In other words Hashirama had no choice much like Hiruzen did. And again without more specific context of what in the world he was into as a kid it turns into death bed regrets. Jiraiya called himself a loser even though he couldn't babysit the Rain Orphans.

Regardless, if the result of cancelling the exams was going to be a few lost Jounin and the invasion being succpered before it could be launched how could you think that Hiruzen intentionally continuing the exam because the evil kid-killer told him to was the right choice?

He was afraid Oro would escalate things, so he left him alone We see nothing, but we have a solid description from Hiruzen. Orochimaru was filled with malice and ambition. It fits with everything we know about him. He develops no compassion - his discussion of Nawaki's corpse and desire to kill the rain orphans fit perfectly with Hiruzen's estimation that the kid was evil from a young age.

Hiruzen doesn't need to give us detail, he tells us exactly how he feels about the whole thing - Orochimaru was malicious and ambitious, and Hiruzen ignored it - which he attributes to the problems caused later. If you're talking deathbed regrets then you're misremembering when Hiruzen admitted to his failures - he wasn't dying at that point. In fact his flashback when he was dying after using RDS was all about when he was made Hokage.

And to add to that, a lot of the characters you mentioned did make obvious mistakes that they should feel bad about. Danzo did horrific things for the sake of the village and sparked several of the issues that Konoha faced, Kabuto literally almost ended the world, Itachi royally screwed up Sasuke and played a part in almost ending the world, Zabuza royally screwed up Haku and Minato decided death was preferable to raising his child.

Because 8-year-old Hashirama who has only known constant war and death shouldn't be expected to be looking out for the psychological health of 8-year-old Madara. The situation changes when you're talking about something Hiruzen, who has known both peace and war and has taken on a guiding role for 3 kids, and 5-year-old Orochimaru who is still infinitely impressionable.
