Just a bunch of excuses, that wouldn't fly for either situation if you're going to be honest.
The only negative thing we see from adult Madara and Hashirama is Madara kicking off about not becoming Hokage and Hashirama putting him down the moment he moves against the village - before then, we see Madara genuinely moved by Hashirama's dedication to peace, so much so that he at least somewhat forgives Tobirama's murder of his sibling.
Hiruzen was literally Orochimaru's teacher, he was responsible for his development. And he ignored a huge negative trait that Orochimaru had which developed into a huge problem for the village Hiruzen claimed to love. On the flip side, Hashirama dealt with Madara the moment Madara made his move against the village.
And before that Hashirama never claimed to have noticed any malice etc. The nail in Hiruzen's coffin came out his own mouth - he noticed Oro had problems and ignored said problems in favour of turning the genius kid into a weapon. Hashirama was a bit off beat and naive but he would know if something is off. Him acting on it is a different story because they formed a friendship. And yet he didn't know that something was off, suggesting that you are incorrect. Hashirama saw the absolute best in people - he honestly was going to give the other Kage weapons of mass destruction for free until Tobirama stepped in.
He doesn't read situations well. Apparently you do, it happens so often in this story that it might aswell be a theme that runs parallel to friendship. Vastly different would mean there are far less similarities between both parties and the scenario than there are differences. We don't have very many examples where the responsible person outright admits that they intentionally ignored their ward's problems in favour of turning them into weapons for war.

It is when you insist a child should be reprimanded for giving off an effed up aura. Sarutobi never went into any detail about what Orochimaru did as a child. Yeah he screwed up big time there. We're still just arguing over when he should have acted. Even if he did notice something was wrong with Madara and he used vague terms, I would have an issue with that and would need more clarification.
Hashirama dealt with Madara in the final showdown. I believe it was stated that they even had periodic battles long into Hashirama's tenure and the final battle was the one Kishi chose to concentrate on. In other words Hashirama had no choice much like Hiruzen did. And again without more specific context of what in the world he was into as a kid it turns into death bed regrets. Jiraiya called himself a loser even though he couldn't babysit the Rain Orphans.
So did Danzo, as did Kabuto, as did Itachi for the 2nd time as did Zabuza way back in the beginning of the story, as did Minato and Kushina oh wow a tandem. This is a common occurrence. That's just it. He couldn't tell the future so This is the same character who acted as if he was taken aback when he finally did stumble onto his former student's hobby.
He's not that good of an actor. Yeah he's a people pleaser, you know what evens that all out? The ability to crush enemies coming by the with many of his own jutsu and having the ability to call forth the 9 tails to take out the opposing village. You know the strongest of the Bijuu by far. Gaara and his father and Chiyo. Sasori and Chiyo by proxy. The 3rd Raikage by way of Ay and Killer B. Kakashi and Sasuke.
There's probably atleast a few more. That is played up for nothing BUT comedic effect. Jiraiya getting off on spying on unsuspecting women is played for comedic effect too.
I don't think you can write it off as acceptable though. Tsunade is forced to live a nomadic lifestyle to escape her debtors. I don't believe we had a situation in the 1st and 2nd's reign that involved Konoha being destroyed by a problem they allowed. Regardless, if the result of cancelling the exams was going to be a few lost Jounin and the invasion being succpered before it could be launched how could you think that Hiruzen intentionally continuing the exam because the evil kid-killer told him to was the right choice?
He was afraid Oro would escalate things, so he left him alone We see nothing, but we have a solid description from Hiruzen. Orochimaru was filled with malice and ambition. It fits with everything we know about him. He develops no compassion - his discussion of Nawaki's corpse and desire to kill the rain orphans fit perfectly with Hiruzen's estimation that the kid was evil from a young age.
It genuinely isn't - even you seem to understand that. Hiruzen doesn't need to give us detail, he tells us exactly how he feels about the whole thing - Orochimaru was malicious and ambitious, and Hiruzen ignored it - which he attributes to the problems caused later. Well I don't think he should've just splattered kid Orochimaru over the pavement and called it a day, but he should've addressed what he saw in Orochimaru rather than ignoring it in favour of making the kid fight a war.
He failed his students in a personal development sense. I'm not sure if their battles escalated to the point that Konoha was in danger until the last one, where Madara took control of Kurama. Madara claims that his fights with Hashirama changed the landscape but the one example we have, the Valley of the End, was formed during the one and only all-out to-the-death fight we see.
And again, the situations are inherently different - Madara is not Hashirama's student. Hashirama doesn't admit he saw Madara's issues and ignored them. If you're talking deathbed regrets then you're misremembering when Hiruzen admitted to his failures - he wasn't dying at that point. In fact his flashback when he was dying after using RDS was all about when he was made Hokage.
And to add to that, a lot of the characters you mentioned did make obvious mistakes that they should feel bad about. Danzo did horrific things for the sake of the village and sparked several of the issues that Konoha faced, Kabuto literally almost ended the world, Itachi royally screwed up Sasuke and played a part in almost ending the world, Zabuza royally screwed up Haku and Minato decided death was preferable to raising his child.
Because 8-year-old Hashirama who has only known constant war and death shouldn't be expected to be looking out for the psychological health of 8-year-old Madara. The situation changes when you're talking about something Hiruzen, who has known both peace and war and has taken on a guiding role for 3 kids, and 5-year-old Orochimaru who is still infinitely impressionable.
